Searches for what a GLP-1 costs through Costco have risen roughly fourfold over the past year. It is an intuitive question — Costco has spent decades being the cheapest place to buy most things, its pharmacy is well known for low cash prices, and it does have a healthcare partnership.
There are two answers, and they come from two different parts of Costco. Costco Pharmacy fills Wegovy and Ozempic pens at $349 for four weeks for a self-pay patient with a prescription, with $199 a month for a new patient's first two months on the lowest doses. The Costco member benefit on Sesame, the telehealth partnership, lists seven discounted services and weight loss is not among them. Novo Nordisk names Costco among the pharmacies stocking its Wegovy tablets; the channels are compared on where the Wegovy pill is sold.

The benefit figures were read from the Costco partnership's own landing page, reached from sesamecare.com/costco, on 2026-09-09 and re-read on 2026-10-06, when the list was unchanged. This page reports what is published and what it costs; it does not recommend any medicine, as our editorial standards set out.
What Costco Pharmacy charges for Wegovy and Ozempic
Novo Nordisk announced on 3 October 2025 that Costco's US pharmacies would sell a four-week supply of Ozempic or Wegovy for $499 to self-pay members with a prescription (NBC News). The price then fell with the manufacturer's own. Sesame's post of 17 November 2025, Costco members can now get half-priced Ozempic and Wegovy (read 2026-10-06), gives the current terms:

The $349 is the manufacturer's price, not a warehouse discount on it. Novo Nordisk's own schedule, read on 2026-09-05 and set out on our Wegovy cost page, charges $349 for every Wegovy pen dose from 0.25 mg to 2.4 mg, $399 for the 7.2 mg HD pen, $349 for Ozempic 0.25 mg to 1 mg and $499 for Ozempic 2 mg, with Wegovy's $199 new-patient offer running through 2026-12-31. Sesame's post quotes one figure for both brands and does not mention the higher-priced strengths, so a patient on Ozempic 2 mg or the HD pen should expect the manufacturer's figure for that strength. The prescription is a separate cost wherever it comes from.
What the Sesame benefit actually lists
Costco's healthcare arrangement runs through Sesame, a cash-pay telehealth marketplace. The partner page is headed "Amazing care. Costco prices." and sets out the member pricing in a block titled "With Sesame + Costco, the health savings add up."

Seven named services and a general discount. There is no weight-loss line, no GLP-1 line, and no medication line of any kind.
This is not a matter of reading one promotional block too literally. The same page's FAQ asks "What kind of perks do Costco members receive?" and answers with its own enumeration — primary care at $29, mental health medication management at $65 a month, health check-ups at $72, virtual therapy at $79, "Plus, 10% off of all other Sesame services, including in-person appointments." The list is given twice, in two different formats, and weight loss is absent from both.
What the Sesame benefit adds to a GLP-1
On the prescribing side, Sesame's ordinary price less 10%. On the medication, nothing beyond the pharmacy price above.
Sesame does sell weight-loss care, and sells it prominently — its site leads with "GLP-1 weight loss, now in pill form," advertising the Wegovy pill "starting at just $149/month," and lists Wegovy pen, Zepbound, Ozempic, Mounjaro and Rybelsus among the medications it handles. That offering is open to anyone. What the Costco relationship adds to it is the residual "10% off all other Sesame services" rather than a named price.
The distinction matters because of how the two figures behave. A named benefit price — $29 for primary care — is fixed and comparable. A 10% discount on a variable service price is neither, and it attaches to the visit rather than to the drug. Nothing in the Sesame benefit reduces the cost of the medication itself; the medication price is the pharmacy's.
Why this is worth stating plainly
The question "what does a GLP-1 cost at Costco" mixes a pharmacy price with a membership benefit, and articles that blur the two quote figures that do not belong together. The first version of this page, read on 2026-09-09, checked only the benefit list and said there was no Costco GLP-1 price; that was wrong for the pharmacy, which was already filling the pens at $349, and it was corrected on 2026-10-06.
The position is easy to re-check: the pharmacy price is the manufacturer's self-pay price, and the Sesame benefit list names seven services, none of them weight loss. A Costco figure much below $349 a month for a branded pen is either the $199 new-patient offer, which lasts two months, or a compounded product, which is a different category.

The parts of the arrangement that are genuinely good
Two things on that page are better than the average cash-pay offer and deserve saying.
Sesame does not bill insurance at all, and is direct about it: "No! Sesame is available to everyone regardless of insurance status, and we do not bill health insurance in order to keep prices affordable for all." Every price is what is paid, upfront, with no deductible arithmetic behind it. For someone uninsured or on a high-deductible plan, a $29 primary-care visit is a real number, not a starting point for a bill.
The cancellation terms are specific, which is rarer than it should be. A full refund is honoured on virtual appointments cancelled at least 3 hours ahead and in-person appointments at least 24 hours ahead; appointments without a fixed time can be cancelled up to 10 days after purchase once the provider confirms the visit did not happen. The exclusions are equally specific: no-shows are not refunded, labs must be completed within 7 days of purchase, and nothing is refunded more than 10 days after the scheduled visit.
One friction worth knowing in advance: the member pricing does not apply automatically. Sesame's FAQ states that members "will need to create an account and verify their Costco membership via their Costco.com account" before any of the discounted prices appear.
What to compare instead
For a reader whose actual question is where a GLP-1 is cheapest without insurance, Costco Pharmacy charges the manufacturer's self-pay price, so the live comparison is between two different things: the manufacturers' own self-pay programmes for approved products, and the telehealth membership models that price a fee and a medication separately.
