Zepbound vials are the same tirzepatide as the pens, in a glass vial drawn up with a syringe — and they are sold only self-pay, only through Lilly's own pharmacy, LillyDirect. Lilly introduced single-dose vials in August 2024; they now come in all six strengths. Their main point is price: the vials and the KwikPen carry Lilly's lower self-pay prices, and the single-dose pen does not.
This page is the device and access record. Label and prices were read on 14 October 2026. It does not describe how to draw up or inject a dose; that procedure is set out step by step in MedibactGuides' Zepbound vial walkthrough.
The vial presentations on the label
The current Zepbound label (Eli Lilly, effective 28 August 2026) lists four presentations. Two are vials:
| Single-dose vial | Multi-dose vial | |
|---|---|---|
| Volume | 0.5 mL, one dose | 2.4 mL, four doses of 0.6 mL |
| Strengths | 2.5, 5, 7.5, 10, 12.5, 15 mg | Four doses of 2.5, 5, 7.5, 10, 12.5 or 15 mg |
| Carton | 1 vial or 4 vials | 1 vial |
| Preservative | None | Yes (same formulation as the KwikPen) |
| Out of the fridge | Up to 30°C for a total of 21 days | Up to 30°C; discard 30 days after first use or after 4 doses |
Source: Zepbound label, sections 3 and 16. NDCs for every carton are printed in section 16.1.
The multi-dose vial was added by a supplement approved on 7 January 2026. When we read LillyDirect's Zepbound page on 14 October 2026, it offered three options — the KwikPen, the single-dose vial and the single-dose pen — and no multi-dose vial. So the presentation most people mean by "Zepbound vials" is the single-dose one, sold as four vials per 28-day supply. Lilly's medical information service states that vials are "available in all strengths exclusively through LillyDirect's self-pay pharmacy channel" (last reviewed 23 February 2026).
What they cost
LillyDirect's prices for four single-dose vials, read on 14 October 2026, with glp1ledger's arithmetic beside them:
| Strength | Monthly price (refill within 45 days) | Regular price | Medicine per supply | Cost per mg (refill price) | 13 fills a year |
|---|---|---|---|---|---|
| 2.5 mg | $299 | $299 | 10 mg | $29.90 | $3,887 |
| 5 mg | $399 | $399 | 20 mg | $19.95 | $5,187 |
| 7.5 mg | $449 | $499 | 30 mg | $14.97 | $5,837 |
| 10 mg | $449 | $699 | 40 mg | $11.22 | $5,837 |
| 12.5 mg | $449 | $699 | 50 mg | $8.98 | $5,837 |
| 15 mg | $449 | $699 | 60 mg | $7.48 | $5,837 |
Prices: LillyDirect, Zepbound page, 2026-10-14. "Refill within 45 days" is the condition Lilly attaches to the $449 tier, which it calls the Zepbound Journey Program. Lilly defines a month as 28 days and four single-dose vials, so a year is 13 fills. Cost per mg and annual totals are our calculations.
Three things the table shows that the price list does not:
- The flat $449 tier makes the higher strengths far cheaper per milligram. At 15 mg each milligram costs about a quarter of what it costs at 2.5 mg.
- The 45-day condition is worth up to $250 a month. Missing it at 10 mg or above raises a fill from $449 to $699; over a year of 13 fills that is $3,250 more.
- A year is 13 fills, not 12. Annual budgets built on 12 months undercount by one fill.
The price excludes needles and syringes, which Lilly says must be bought for every weekly dose, at checkout or elsewhere, and are different from KwikPen needles. Lilly's terms say the savings are "subject to monthly and annual limits" and can be changed "at any time without notice". The full price comparison across Zepbound's presentations and channels is on Zepbound cost; LillyDirect itself is described on our LillyDirect page.

Eligibility and what the route requires
Lilly's vial footnote reads "Self-pay only. Eligibility required, terms and conditions apply" and links to full terms. The list of government programmes printed on the same page — Medicare, Medicaid, Medicare Advantage, Medigap, the Department of Defense, the VA, TRICARE and state assistance programmes — sits under the single-dose pen's savings card; we did not open the separate self-pay terms, so this page does not state whether that list also applies to vials. The route also requires a prescription for Zepbound itself: Lilly's medical information page states that tirzepatide is only available through LillyDirect "under the Zepbound brand name", which is why a vial of compounded tirzepatide is a different product with a different legal status — see compounded tirzepatide vs Zepbound.
What is not established
- Whether the multi-dose vial will be sold. It is approved and on the label; Lilly's pharmacy page did not offer it on 14 October 2026.
- How long the current prices last. Lilly's terms allow change without notice; the figures here carry their read date.
- Equivalence in use. The label treats the presentations as the same medicine; the trials behind Zepbound's results used the single-dose pen.
Which presentation fits a given person — vial, KwikPen or pen — depends on their prescription, coverage and ability to handle a syringe, and is a conversation for their prescriber and pharmacist.
