LillyDirect Explained: What Lilly's Own Pharmacy Sells, Who Fills It and What It Costs

LillyDirect is Eli Lilly's direct-to-patient service. A prescription goes to LillyDirect Pharmacy, LLC in Indianapolis, and one of four partner pharmacies — Gifthealth, FuzeHealth, Amazon Pharmacy or Walmart for pickup — fills it. The medicines, the self-pay prices for Zepbound and Foundayo, the 45-day refill rule and what LillyDirect does not do, dated.

LillyDirect is Eli Lilly's direct-to-patient service — a website, a pharmacy of record and a set of partner pharmacies through which Lilly sells its own medicines, including Zepbound and Foundayo, straight to people with a prescription. It is not a telehealth company and it does not prescribe. This page sets out what it is, who actually fills the order, what it charges and where its limits are, from Lilly's own pages and one federal register, all read on 30 September 2026.

Who is behind it — the register entry

The pharmacy prescriptions go to is LillyDirect Pharmacy, LLC. The federal NPI registry lists it as:

Lilly's Zepbound page gives the same NPI, an NCPDP number (1574056) and the same address for prescribers.

The medicines are then dispensed by third-party pharmacies. Lilly's prescription-support page names four:

  • Gifthealth — home delivery
  • FuzeHealth — home delivery
  • Amazon Pharmacy — home delivery
  • Walmart Pharmacy — in-store pickup

So a LillyDirect order is a Lilly-run front door and pharmacy of record, with a partner doing the filling and shipping.

*Only "when you refill within 45 days". Otherwise the regular prices apply: $499 for 7.5 mg, $699 for 10, 12.5 and 15 mg. The KwikPen needs pen needles and the vials need needles and syringes, bought separately. Lilly's medical information service states the vials are "available … exclusively through LillyDirect's self-pay pharmacy channel". The single-dose pen is sold through LillyDirect's insurance channel, where the savings card can bring a covered fill to as little as $25.

How an order moves, and how long it takes

Lilly publishes the steps after a prescription reaches LillyDirect Pharmacy:

  1. Within 1 business day — a text to confirm details and "determine the right path".
  2. Within 2 days of confirmation — a status update.
  3. Within 2 business days of the update — a checkout link.
  4. Within 3 business days of checkout — shipment with a tracking number.

Added up, that is up to about eight working days from prescription to shipment, before delivery time. For a Zepbound user on 7.5 mg or above, that sits inside the 45-day refill window only if the next order starts well before the pen runs out.

LillyDirect also says it checks insurance "routinely" and supports prior authorization when a plan requires it — the process is on prior authorization for GLP-1s.

The telehealth listings

LillyDirect's home page lists independent telehealth services for obesity, with the visit prices they report — from about $25 per visit with insurance to $29–$299 per visit or $99–$149 a month self-pay, read on 30 September 2026. Lilly states: "Lilly does not endorse or recommend these health care providers", and that the rates "do not include medication costs". The providers decide independently whether to prescribe. The drawbacks, stated plainly

  • Lilly products only. A person comparing Zepbound with Wegovy cannot do it inside LillyDirect.
  • The top-dose price is conditional. Miss the 45-day refill window and 10 mg goes from $449 to $699.
  • Extras cost extra. Needles for the KwikPen, needles and syringes for vials.
  • Time. Lilly's own steps allow about eight working days before shipment.
  • Cash and insurance do not mix. Lilly's card terms bar seeking reimbursement from insurance for any card savings; whether a plan counts a cash purchase towards its deductible is a question for the plan.
  • Terms can change at any time. Every price above carries Lilly's statement that the offer may be modified or ended "at any time".

What it is not

LillyDirect is not insurance, not a prescriber and not a compounding pharmacy. It sells branded Lilly products only, so it is also not a source of compounded tirzepatide — the difference is on compounded tirzepatide vs Zepbound.

How much is Zepbound through LillyDirect?

Self-pay, read on 30 September 2026: $299 a month at 2.5 mg, $399 at 5 mg and $449 at 7.5, 10, 12.5 and 15 mg for the KwikPen or four vials, with the $449 conditional on refilling within 45 days. With commercial insurance that covers the single-dose pen, Lilly's card offers as little as $25 per fill, with a $100 monthly cap.

Is LillyDirect cheaper than other pharmacies?

For the Zepbound KwikPen, Lilly's savings page shows the same $299 starting self-pay price at LillyDirect and at other pharmacies. Vials are only sold through LillyDirect. Prices at an ordinary pharmacy using insurance depend on the plan. What differs is the route, not the list of self-pay prices Lilly publishes.

What are the drawbacks of LillyDirect?

It sells only Lilly products; the lowest top-dose self-pay prices depend on refilling within 45 days; needles and syringes for the KwikPen and vials are bought separately; Lilly's published timeline allows about eight working days before shipment; and Lilly states that the telehealth providers it lists are independent and not endorsed by it.
