Foundayo Cost 2026: Self-Pay Prices, the Savings Card's Caps and What a Year Really Costs

Foundayo (orforglipron) costs $149 to $349 for a 30-day bottle self-pay, with two temporary reductions and a $299 refill offer on the top doses. The commercial savings card brings a covered fill to as little as $25 but stops after 10 fills a year. Every price read from Lilly's terms on 2 October 2026, with the yearly arithmetic.

Foundayo (orforglipron) is Eli Lilly's once-daily weight-management tablet, approved on 1 April 2026. Its price is set by Lilly's published terms, which carry a regular ladder, two temporary reductions, a refill offer on the top doses and a savings card with caps that are tighter than they look. This page reads all of them from foundayo.lilly.com on 2 October 2026 and works out what a year costs.

Self-pay prices by strength

Every price is for a 30-day bottle of 30 tablets.

The refill offer. Lilly applies the $299 price automatically on the first purchase of 14.5 mg or 17.2 mg. "To continue to be eligible … you must complete your refill purchase … within 45 days of the delivery/received date of your previous Foundayo prescription fill." Miss the window and the price returns to $349. A 30-day bottle leaves 15 days of slack before the window closes.

If the discount is not applied at the pharmacy. The terms allow a post-transaction reimbursement claim when Lilly "was not able to verify such at the point of sale", through an online portal or by post, with the original receipt — postmarked no later than 31 March 2027.

Lilly offers these prices through LillyDirect and through "other pharmacy"; the partner pharmacies behind Lilly's direct-to-patient channel are listed separately. Lilly's page also marks the self-pay price as HSA/FSA eligible, subject to the plan's administrator.

A year of Foundayo, in dollars

Monthly prices hide the fact that a "month" here is 30 days, and that nobody stays on one strength forever. At a single strength for a full year:

The $299 top-dose price puts Foundayo level with the Wegovy pill's $299 price at 9 and 25 mg — the same $3,638 for a year — read in full on Wegovy pill cost. The label's dose steps are at least 30 days each, so a first year includes lower-priced months; the exact total depends on how long a person stays at each strength, which is the prescriber's decision.

The patient pays $25 when the plan's copay for a 30-day fill is $125 or less, and the copay minus $100 above that. The yearly cap is exactly ten monthly caps: 10 × $100 = $1,000.

The ten-fill limit is the binding term. At one bottle per fill, ten fills cover 300 days, leaving 65 days of the year outside the card. Lilly's Zepbound and Mounjaro cards allow 13 fills; Foundayo's allows 10 (compare the Zepbound card's 13-fill terms and Mounjaro savings card). Because a 2- or 3-month fill counts as a single fill, longer fills stretch the limit — but the $1,000 cap still applies, and a plan may not allow a 90-day fill.

The savings card when insurance does not cover Foundayo

For people with commercial insurance that excludes Foundayo, the card sets a price by strength and a maximum monthly saving. Add the two and every strength comes to the same number:

glp1ledger arithmetic from Lilly's terms. A cap is a ceiling, not necessarily the pharmacy price, so $669 is the reference price the card is built around rather than a price Lilly publishes.

So the card discounts from a flat $669 for a 30-day bottle at every strength, the way Lilly's Mounjaro card terms imply $1,146 for a 28-day pen fill (Mounjaro cost). This tier is also limited to 10 fills a year, and its top-dose $299 is subject to the same 45-day refill condition.

Who is excluded

Both card tiers require a prescription "for an approved use consistent with FDA approved product labeling" — Foundayo's label is for chronic weight management in adults with obesity, or overweight with a weight-related condition — commercial drug insurance, age 18 or over and US or Puerto Rico residence. They exclude anyone enrolled in "Medicaid, Medicare, Medicare Part D, Medicare Advantage, Medigap, DoD, VA, TRICARE®/CHAMPUS, or any state prescription drug assistance program". As with Lilly's other cards, plans running an alternate funding programme make their members ineligible, and the card cannot be combined with any other discount.

Medicare

Lilly's page lists Foundayo at $50 a month through the Medicare GLP-1 Bridge "for eligible Medicare Part D patients with Foundayo prescription for weight management", with eligibility "based on Medicare GLP-1 Bridge Clinical Criteria" and "not a guarantee of coverage". What the terms do not say

  • Anything about 2027. The card and both temporary reductions end on 31 December 2026.
  • Whether a plan covers Foundayo. That depends on the plan's formulary; see GLP-1 insurance coverage.
  • Whether the refill offer survives a dose change. The terms tie it to buying "your refill" of the 14.5 or 17.2 mg dose within 45 days and say nothing about moving between those two strengths.

Lilly's regular self-pay ladder has four price points for six strengths: $149, $199, $299 (5.5 and 9 mg) and $349 (14.5 and 17.2 mg). The temporary reduction moves 5.5 mg to $199 until 31 December 2026, so during that period 9 mg is the first strength at $299.
